The below questionnaire will be reviewed by a Perch Clinical Supervisor to ensure we are able to individualize an enjoyable and enriched summer learning experience for all participants.
A clinician will get in touch via email or phone within 2 business days with confirmation of enrollment availability and instructions to submit payment information.
A contract for financial responsibility is due upon confirmed enrollment.

07/08/2024: We are again accepting some 1:1 campers for Week 5 and 6 at Graham Hill. Space is extremely limited. All of July is now full for Roxhill; Week 3 at Graham Hill can accept 1 or 2 campers. Space is still open for Weeks 5 and 6 at both camp locations.
